- Patients that have an appointment with a specialist, such
as
Endocrinology, Cardiovascular Surgery, or Nephrology, should have the
proper
authorization or referral forms completed by their primary care physician
prior to their appointment. Charges associated with unauthorized services
will become the patient’s responsibility until the proper authorization is
obtained by the patient and provided to the specialist office.
- Please
present your insurance card to the receptionist at each appointment.
Verification of your current insurance coverage is crucial when filing a
claim
to the insurance carrier.
- Copay amounts are due at the time of
your
service and should be paid when you checkout from your appointment. Mid
Michigan Physicians accepts cash, check, or credit card
payments.
- There
are many commercial insurance carriers that Mid Michigan Physicians do
not
participate with. As a courtesy Mid Michigan Physicians billing
office
will submit a claim to your commercial insurance twice, if after two
submissions the services remain unpaid the charges will be transferred to
patient responsibility. We will send you a letter and a copy of the claim
that provides the information needed for your insurance to process the
claim.
Upon receipt of the letter you should make payment to Mid Michigan
Physicians
for the services and submit the claim to your insurance company for
reimbursement.
- Mid Michigan Physicians mail patients
statements
on a
monthly basis. Payment is due upon receipt of your statement. Please
make
check payments out to Mid Michigan Physicians, P.C. and include the
patient account number on your check.
- Patient accounts with
balances
that have aged 90 days without payment will be transferred to a collection
agency for processing. If you are receiving statements and feel the
services should be covered by your insurance please contact our office and
discuss your account balance with a Billing Specialist.
